diagnosis for Mohs closures

what diagnosis do you use for mohs closures. If the basal cell has been removed by the derm and the plastic surgeon is closing it do you use open wound nose (if it was on the nose) or a v code for history of skin cancer?

We always used an aquired deformity of the nose with the hx of neoplasm code.
