Major coding issue at our office. We have a patient that is coming in for INR only. Being coded as 434.10, 453.79 and 436. I do not agree.
Last office visit in August 2010 states thrombosis was in 08 and pt is now doing well with no problems. CVA was in 03 and also states pt doing well, no complaints. Also states pt here for anticoag management/follow up.
Would this not be coded as pers history of and not acute codes? As well as the V58.61?

This is happening with all of our follow up patients. All are being coded as current problems and not history of. Please help, I do not want to bill incorrectly. No one seems eager to help.
