Wiki diagnosis question for Head CT

Patient presents with altered mental status. The body of the report reads that there is encephalomalacia seen at site of prior right MCA infarct. The impression reads manifestation of old right MCA infarct.

Would it be correct to code the encephalomalacia as the primary diagnosis?

I think we can use it as pdx. I've another question. encephalomalacia due to old infarct- can we use both 434.90 and v12.54 in this scenario?
