patient saw dr and he was diagnosed with S83.232A - COMPLEX TEAR INITAL ENCOUNTER, he plans for surgery and has 29881 - arthroscopy surgery and the dr. puts S83.232A again, for the diagnosis, My question is should this be a D instead of an A? thank you

No. The A is appropriate as long as active treatment is still going on--which will likely be all the until surgery is completed. Afterwards, when the patient returns for followup, you can transition to using the D.
