If a providers impression of a x-ray of a joint just states "degenerative changes" or "osteoarthrtic changes" can you code arthritis or osteoarthritis from that? or just code the indication for the test which is usually pain
 
I would not code the arthritis, either query for clarification or code the indication. from the statement you provided there would be no way to know if it is an osteoarthritis or if it is primary or secondary.
