What specifically about the rules are you looking for clarification on?

The supervision level for every covered CPT and HCPCS code is indicated on the physician fee schedule - if you look up any code in the PFS (https://www.cms.gov/medicare/physician-fee-schedule/search) and scroll to the right find the column labeled 'Phys Supv' and you'll see the indicator for that code.

What is the meaning of 09-concept doesn't apply? I have looked everywhere for an explanation and example of this.
