I hope someone can help us with this confusing question!! If a Nephrologist has been referred to do a consult to manage dialysis for an ESRD patient admitted to the hospital for other reasons than renal, can you code for a consult (99254) and dialysis (90935)? We tried using modifier 25 with the E/M code but it has been getting denied. Medicare is paying for the dialysis (90935) only.
 
You cannot bill a consult and dialysis on the same day. Even with a modifier. I do not actually do Nephrology billing - one of my co-workers does. If I am not mistaken you can code an admission and discharge with dialysis but not visits or consults. Hope this helps!
