I am completely confused on how to code this and I've been coding for 16 years. This pt had a previous ventral hernia repair with closure of diastasis recti. This time he has mesh removal (20680), bilateral myofacial flap creations (15734x2), LOA (included), and so much more. Asking for HELP!?!?!? How would you code this note? TIA!!!
