On an OP, note information is noted under OPERATION PERFORMED, but NOT in the BODY or FINDINGS AND PROCEDURE of the note; can that information be coded?

I always code only from the the body of a procedure note, also findings if it is relevant to the picture, but never from the operation performed. The body of the note is what tells you what procedure was done.
