I am a procedural coder for Cath lab cardiovascular cases.
My question is can any portion of procedural charges be
captured from the procedure log?
Example LV pressures not dictated by physician but
resulted and captured in the procedure log by clinical staff.
This was not indicated in the physicians documentation
to reference the log.
