differential diagnosis on office visit vs procedure note

Can someone please help? I was curious about what people thought about the diagnosis listed on the operative report vs the office visit. Patient comes in to ENT and dx is Chronic OM. Patient is scheduled for Tubes. The dx on the op report is Chronic Eustachian Tube Dysfunction only. I know that the OM is more than likely caused by the ETD, but shouldnt the office visit dx and the OP note dx match. Or does that matter?
