Wiki Discharge Day vs Subsequent day and prolonged care

Does anyone know if a physician can code for a subsequent day and prolonged care code instead of a discharge code on the day the patient is discharged from the hospital? If they can or can not....would you have the information site where I can show this to the physicians.
