Discharge from hospital to nursing home

We have a patient that was seen by their PCP while being an Inpatient that was being sent to a nursing home. Patient was suppose to be discharged to the nursing home but when the PCP went back to the hospital the next day she ended up seeing the patient again (for an extended period of time) because the nursing home had not come and gotten her yet as a bed was not ready for her. The PCP documented a discharge on the day before but is now seeing the patient again ?? anyone have any input on what they would suggest for the billing of these services?
