Discharge from Hospital

I have a question: When a patient is discharged from the hospital and they can't leave because they are waiting for Awaiting guardianship, placement and doctors continue to bill subsequent visits. I know you can't bill the subsequent visits following discharge because it's not medically necessary but where can I find this info that is detailed? I looked at the cms manual and it's not very specific. Can anybody help on where I can find info on this please?
