I wanted to clarify...I understand that if the physician documents both an discharge from Inpatient and then an H&P to SNF the same day that both 99238 and 99304-99306 could be billed for the same DOS. However, when the physician does not document at a minimun detailed hx/physical he can not meed the criteria for a 99304. Would he only be able to charge for 99238 discharge since components are missing for the initial SNF admit?
