When pt last seen the day before DC Summary filed date how do you bill for DC?

Last available progress note is 3/24 which physician signs stating DC to occur on 3/25 if no changes. The DC Summary note time is 3/24 1330. The Filed date is 3/25 and dictated from the 3/24 progress note. The physician did not see the patient on 3/25. The appropriate date for billing DC is?
 
Professional or facility?

My experience is strictly for professional coding, so that's what I will answer.

In your example, the physician may NOT code anything on 3/25 because physiican did not see the patient that DOS.

Physician wil use the subsequent hospital visit code on 3/24 when patient was actually seen/examined.
