My ophthalmologist has asked me to help him with his ICD-10 transition. As you can see, I am employed by a local hospital as a Coder. I?ve explained to my ophthalmologist that I am more than willing to help him but that I do not represent the hospital in this, and that I?m giving this information to the best of my knowledge as a CPC. He does understand, but I?d still like to attach a ?Disclaimer? to anything I give him?? Could you direct me to a good one that is all encompassing and yet doesn?t sound too abrupt?
