Hi there, I have a provider that attempted to perform an AV ablation via the femoral artery with no success gaining access. Moving on to attempt from the Radial access was still unsuccessful so the ablation was aborted. Further, the patient was to have an upgrade to a BiV system but due to subclavian stenosis the catheter couldn't move through the artery for successful lead placement. The patients pocket was however revised for the larger BiV device but the old device was reinserted. Patient was under anesthesia for almost 3 hours.

I'm not sure how to code this! Any help would be appreciated.
