Patient is scheduled for AVR and CABG. They do the sternotomy, harvest the internal mammary artery and a saphenous vein. The pericardium is incised and somewhere along the line pursestrings were placed in the aorta and the right atrium.

The decision is made to discontinue the surgery due to the risk of continuing it being higher than not, they even had to do manual cardiac compressions at one point.

So how do you code this? Do you code out the lowest level CABG involving both arteries and veins as being discontinued or do just code out the AVR as discontinued or do you code all of them?
