dispensing diabetic shoes and orthotics

Can orthotics (L3000) be dispensed by a nurse or MA? Also can someone other than the podiatrist cast for the orthotics?

I'm pretty sure that the Podiatrist is the one that has to dispense the diabetic shoes and inserts for fitting purposes, etc, but what about casting?
