Looking for direction on this one:
The urethral meatus appears punctuate. Progressive dilatation is performed up to 22 Fr using progressive metal sounds. A 21Fr rigid cystoscope was introduced into the urethra and advanced pendulous urethra with some resistance. This also appears narrowed and pale all the way down to the urethral sphincter. The prostate is symmetrically enlarged w/o median lobe. The bladder is trabeculated. No stones or tumors were noted into it. At this point, the cystoscope was withdrawn, and a 26 Fr metal sound was introduced to stabilize the distal urethra. Using a 15 scalpel a triangular section of the meatus was excised in its ventral aspect, with the base of triangle being proximal. This allowed to remove a large amount of scar tissue. 4-0 chromic interrupted sutures were applied bilaterally to reapproximate the urethral mucosa to the ventral plate.
