IDDM is listed in PMH but states borderline DM normal glucose, in assessment.
would you code the diabetes?
 
I would ask the provider to clarify. You don't want to give someone DM if they don't have it.

Better safe than sorry. This could take years to get corrected if at all with the insurance company and could cost the patient a lot of money in premiums and possibly get them dropped from coverage all together.
