How would you code this on an outpatient wound care notes:
HPI documents: Mr XX has 2 ulcers of the right lateral ankle region.
Impression documents: "Diabetic wounds of the right lateral ankle in the setting of peripheral arterial disease"

Would you code?: 707.13, 250.80 and 443.9 (PAD)
I did not connect the PAD to DM as it did not state "due to DM".
In your opinion(s) would you agree?
