I have two part question

diabetes with visual loss - better. is listed in Problem List

The assessment only lists Diabetes.

They coded 250.50.

I understand the Problem List for Risk Adjustment can be used as long as there is indication it was addressed in this visit.

I'm not sure what to code because the assessment is only diabetes 250.00
