Wiki DM with manifestations - assessment?

If a provider appends a dx of, let's say, DM 2 with CKD 3, and then in the body of the note specifically addresses only the DM 2 (i.e., "DM 2 under good control with current medications"), is it acceptable consider the CKD has also been addressed? Said another way, if a provider assesses the status of DM which is associated with manifestations, are the manifestations also being assessed? And does anyone know of any official guidance on this topic?
 
Does he say DM 2 WITH CKD3 ? If he does not in some way relate these two then the CKD is not a manifestation of the DM. So if he in fact states it this way the I do assume both were addressed.
