If you have DM and the Dr indicates the patient has an ulcer or hypoglycemia but does not link the conditions...should you code to 250.80 since it is inherent? Please help!

For ulcer:
Ulcer should not be linked to DM unless the provider states so, since the ulcer could be due to other underlying condition
For Hypoglycemia:
When you index hypoglycemia icd9 251.2 and then verify it in the tabular list, a notation: "Excludes hypoglycemia in diabetes mellitus (249.8, 250.8X)" directs you to assign DM icd9 250.80
In this case, your codes should be:
250.80+707.9
