Wiki DME POS for OP Office Visit

I know on normal DME the POS would be 12 but if a patient is seen at an outpatient clinic inside of a hospital and is given a piece of DME equipment (like a wrist brace), would that be billed on 2 separate encounters...one for OV and one for DME? If so, what POS would we use for the DME? Can we bill POS 11 for all services?

I also know Medicare and Medicaid require POS 12 regardless but I was not sure about commercial. Thank you!
