Today I was coding an Observation stay with the ER visit. We combine these services on one account, so I code them together which usually isn't an issue. However, this time the ER doctor diagnosed the patient as having COPD/Chronic Bronchitis and Pnemonia and the admitting doctor diagnosed the patient in the H&P with Acute Bronchitis. Usually I queiry the physician and document the solution. Does this ever happen to y'all and what do you do?
