How much documentation is required for a non-covered service? For example; a new patient coming in for a procedure which is not covered by Medicaid. Nothing will be billed to insurance. Thanks!
 
Ethically, the provider should still document as if the service was covered. Documentation should not solely rest on whether a service is covered/being paid or not, but what is medically necessary. The provider should know this as part of their training.
