Ahh yes the CPT code 99401-99404 demand minutes be documented in med record for date treated and linked to a preventive problem such as diet, sTD sex, or substance abuse (booze or sub abuse drugs cocaine, pot weed,ETC ).But these CPT cannot be done same time as MH or Health Assessments of CPT 96156 till CPT 96171 .You could do a Eval mgt CPT99202-99214 visit, then add modifier 25 then CPT 99401-99404 minutes added with specific dx.
