We are billing 76937 26 for US guided procedures that are performed at an Ambulatory Surgery Center (ASC). Is the provider's office responsible for ensuring that a permanent record of that US is kept at the ASC before billing? If a permanent record is not kept, is the provider not able to bill his professional fee?
