Hello,

Provider ordered a Depo for a pt. But there is no documentation in the chart that depo was given, however it is located in the MAR. Is this billable?

Not sure what you mean by MAR. Is that part of the medical record? if not, you may not bill without documentation of the medication (with the dosage, route of injection, NCD#, and physician order) documented as well.
