Looking for quidance or suggestion on what needs to be documented in order to bill for 29823.

Look at the article July 2017 Healthcare Business Monthly. For your answer. It gives several very specific examples.

Remember, any debridement that is part of a restorative procedure (Rotator cuff repair, SLAP repair, biceps tenodesis) is NOT counted towards 29823. It has to be debridement that is not related to a procedure.
 
There was an article in the July Healthcare Business Monthly regarding shoulder procedures ( Page 36 ) It states extensive debridement includes debridement of multiple soft structures, multiple hard structures, or a combination of both. an example would be a chondroplasty and a debridement of the labrum( a combination of hard and soft structures).
It can also include removal of osteochondral and /or chondral loose bodies, biceps tendon and rotator cuff debridement, and abrasion arthroplasty.
