Wiki Documentation for Have to Bill with ICD9 & who provide codes?

I have been asked to provide documentation on where it is written that we must bill with ICD-9-CM codes, who can provide the codes and if a coder is necessary or optional for companies making Medicare bids. Anyone have any ideas?
