Hello,

Could anyone give an example on what proper documentation would be for the requirement of a written screening schedule for the next 5-10 years for beneficiaries when performing a Medicare Annual Wellness Visit? Right now I have a provider that documented the following:

"We discussed the risks and benefits of the following screenings and immunizations based on USPSTF AND ACIP guidelines:

Screenings:
Colon Cancer screening addressed

Immunizations:
Immunizations reviewed and discussed, but declines Influenza vaccines.

Is this enough? The 5-10 years is throwing me off. PLEASE HELP!
