Hi Everyone!

I'm going back and forth with a provider requiring the documentation they are sending for pool therapy services.

The services are coming on a form which states across the bottom, "Pool/Program Progress Note", however, the entries for the specific dates of services don't mention pool therapy at all.

I don't think this is sufficient documentation. The provider maintains that having this is more than enough to consider reimbursing the services.
