Need some help in locating some documentation guidelines regarding the specific documentations when its a " foreign body removal"


I have specific providers from ENT that I am auditing whom I noticed they do not document the specific type of FOREING BODY that is being removed. Can someone help me in finding something set in stone if any in regards to documentation guidelines for foreign body specifications?

Can they bill for foreign body removals if they don't specify what kind or type of foreign body is being removed?
