Wiki Documentation guidelines for Hemorrhoid banding?

Can anyone point me in the direction of a site that will give documentation guidelines for what is necessary to have been done prior to performing a hemorrhoid banding? I know that the patient has to have failed conservative treatment and this procedure shouldn't be the first line of defense in treating the hemorrhoids but how much prior documentation is required?
