Does anyone know if the documentation requirements are the same for par and non-par providers. Currently I am concerned with Horizon, but I will take any information that you may have.

I'm gonna say yes. You shouldn't be documenting for an insurance company, but for whatever CPT you are billing. As non-par they don't have to pay you, but are probably doing so as a courtesy to their member. They can still ask for records to justify what you are asking payment for.
