Can anyone provide me with a description of the documentation required/expected for H0018? PLease?
 
In my experience, the HCPCS codes in this range are accepted by very few payers, usually state Medicaid plans. I'm not aware of any general coding/documentation guidelines for these, so you'll probably need to review the policies for the specific payer that you're billing to get this information.
