trying to assist our providers with the requirements for documenting the moderate sedation during a procedure (cardiology specialty). does anyone have the list of requirements or a template for such requirements, or perhaps a sample of the documentation used to support the moderate sedation codes. any help would be appreciated
 
I am looking for this as well. Historically we just uploaded the hospital records to show sedation time. But I would like the physician to document it in his report. Does anyone have a sample or template of the wording their doctors use?
