I have read a lot about what is not acceptable for the physician documentation to support a Split/Shared Visit but have not seen examples of what is acceptable. Can someone share/give pointers of what is acceptable for the physicians documentation.

IP OR OP. ? in OP you have to follow "incident to" rules. for IP the MD &Should document that they performed at least one complete part of the E&m. both must sign. often the md just puts an addendum to the mid levels note. you need to check payers for special rules.
