The provider specifically documented just SDH and did not mention LOC or without.

Would the correct code be S06.5X0A-traumatic subdural hemorrhage without loss of consciousness?

OR S06.5X9A (NOS)-traumatic subdural hemorrhage with loss of consciousness of unspecified duration?

Are there documentation requirements for with or without LOC?
