Hi everyone: I am wondering if anyone has information on documentation requirements to support the coding of a Manifest Refraction. Some of my optometrists have changed how they are documenting their Manifest Refractions. To stating: Manifest refraction same as lens prescription" or "Refraction: Manifest then they have the spectacle prescription documented." I'm in the middle of formal audits and stumbled on these 3 optometrists who have completely changed how the document their Manifest Refractions. Thank you in advance for any help and information!
 
In general, the refractions are non covered services by most insurers so whatever the ODs are using to document the MR shouldn't be a factor in your audits. As long as they have the final RX they want to prescribe for a patient and it's readily apparent to anyone who looks at the file, that should be sufficient.
