I work with a therapy group that is considering offering OT, PT and ST services for free for a summer camp. What documentation is required? I believe they still need to have consent to treat form, physician referral forms and a treatment plan as well as clinical service notes. Anyone have any ideas where I can find guidance on this?
 
I would start with your State laws and department of health requirements. While it doesn't sound like you will be submitting claims to a payer, most states have requirements for medical professionals to provide care. Next I would check with the provider insurance coverage. There may be malpractice insurance clauses that need to be addressed as well. I agree with you regarding consent to treat as both protection for the patient and provider. Hope this helps.
