Can anyone provide a link to something in writing from a credible source stating what the medical record documentation requirements are for patient identifiers on a test or document to properly identify a specific patient. Just the patient name, the patient name and ID#, etc. I know I have read this somewhere, but I need to be certain our office is in compliance when we identify a test and scan it into a patient record.
