If a provider documents a Flu Vaccine on their paper encounter form, but I can not find it in the Electronic Medical Record is it ok to ask them to amend their record to reflect the immunization?

If you scan your paper documents into the EHR, that would be a better method, as that way there are no late entries. If not, then do you have a policy on late entries into your EHR?
 
Thank you so much! That is such a great idea! I was just so on the fence about them amedning the record months after the service that I never looked at what was in front of me for proof. :D
