Morning, We have contracted BH provider that is a PMHNP-BC and she is off site but sees out patients on site, does she have to document in her encounter that it was a telehealth visit?

Yes she should document if video or phone & amount the minutes in the documentation of illness. Also she should state verify this is correct patient, assessments and medication and describe pt problem. Phone CPT 98966-98987 phone by QHP and nurses. CPT 99441-99443 MD, NP PA DO. Or if use mental health CPT 90832-90837 90791 add modifier 95 video or modifier 93 for phone. Tell your PMHNP to view dx codes of dx R45, F32, F33 and if a substance abuse problem be specific if possible.
