Wiki Documenting AWV Required Elements

There are required elements for the Medicare IPPE and AWV. Do each of the required elements need to be documented in the progress note specific to that visit, or is it sufficient to have some of the elements recorded in another location of the patient's electronic chart, as long as updated on the same date of service?

For Example: the requirement "Establish or update a list of current providers and suppliers - Include current providers and suppliers that regularly provide medical care to the beneficiary", our EMR has a field called "circle of care" where a patient's current providers are all listed, but this field is not attached to the progress notes. Is it sufficient for Medicare requirements if the progress note states "see Circle of Care" under the requirement for listing current providers?
