Wiki documenting time

How would you handle a situation when you know that the time the physician is documenting is not correct. Ex: documents that the visit was from 10-10:55 but it was really 10-10:30. Thanks

Documenting that the visit was from 10:00 to 10:55 does not qualify for counseling time. The physician must specify that more than 50 % of the visit was spent on counseling and must document the accurate total time and total time spent counseling. You will need to ask for clarification from the physician.

Our problem is that she states incorrect time with every visit. If we total the time that she documents she would work 18-20 hours everyday. Thanks
